Emergency Medical Call Reports
Patients: To request a copy of your report, you must present photo identification in person at Fire District 1 Headquarters, 12425 Meridian Ave., Everett WA 98208.
Attorneys and other agencies: To request a report on behalf of a client, submit a written request on letterhead along with authorization to release medical information signed by your client. Requests can be submitted:
