Requesting Public Records

Emergency Medical Call Reports
Patients:
To request a copy of your report, you must present photo identification in person at Fire District 1 Headquarters, 12425 Meridian Ave., Everett WA 98208.
Attorneys and other agencies: To request a report on behalf of a client, submit a written request on letterhead along with authorization to release medical information signed by your client. Requests can be submitted by mail to Fire District 1, 12425 Merdian Ave., Everett WA 98208 or by fax to (425) 551-1260.

Fire Reports
Submit request in writing to Fire District 1 Assistant Chief Brad Reading, phone (425) 551-1257, fax (425) 551-1234.

Requesting other public records
Contact the Public Records Officer Leslie Hynes at  (425) 551-1243, to request other public records from Fire District 1.
